Causes of Chronic Buttock Pain: Is it Piriformis or a Disc Issue?

Updated: May 6

If you are suffering from chronic buttock pain, you have likely been told you have "Piriformis Syndrome" or a "tight hip." While stretching may provide temporary relief, the pain often returns because the source of the nerve irritation hasn't been fully addressed. At Triangle Spinal Decompression, we look deeper into the clinical reality of your symptoms, specifically investigating a phenomenon known as the "Double Crush."

Chronic Buttock Pain: The Double Crush Phenomenon

The "Double Crush" theory suggests that a nerve can be compressed in more than one location. For many Cary residents, chronic buttock pain is actually a two-part mechanical failure:

  1. The Primary Crush: A lumbar disc herniation or bulge (often at L5-S1) irritates the nerve root at the spine.

  2. The Secondary Crush: The nerve, already sensitized at the spine, becomes easily entrapped by the piriformis muscle in the buttock.

If you only treat the hip, you are missing the engine of the problem. If you only treat the spine, the secondary muscular entrapment remains.

Spinal Decompression

A Comprehensive Clinical Solution

Because our clinic is located within Swank Chiropractic Sports Medicine, we are uniquely equipped to resolve both ends of the Double Crush. We don't just "crack backs" or rub sore muscles; we provide Biomechanical Restoration through an integrated protocol:

  • Spinal Decompression (Triton DTS): We utilize the Nutrient Pump Effect to reduce the "Primary Crush" at the disc level, rehydrating the disc and taking pressure off the nerve root.

  • Dry Needling & FAKTR: Dr. Parker Neill utilizes these specialized techniques to release the "Secondary Crush" within the piriformis and surrounding soft tissues.

  • Functional Stability: Dr. Abigail Swank focuses on the long-term stabilization of the core and pelvis to ensure the biomechanical failure doesn't return.

Integrity-First Diagnostics

We believe in a "diagnosis-first" approach. Before beginning any care plan, we perform a definitive clinical assessment to determine if you are a candidate for our protocols. We prioritize evidence-based outcomes over high-pressure sales, offering elite-level care at fair-market rates for the Cary community.
